Foundations 2 Change

Foundation 2 change is a recovery service, who prides itself on supporting prisoners, care leavers and families to find hope that change is possible, utilising our proven techniques of support to empower the individual to lead a fulfilling life.

We support people to recover from mental health, addiction, homelessness, offending and social isolation, reconnecting them back within their community.  

Foundations 2 Change support individuals leaving the prison service, mainly in HMP Birmingham. We are a prison and community model delivering transition support which maintains continuity of care. We have community sessions that embed prisoners into new positive peer groups where they continue to build recovery capital and receive ongoing support from the Foundations 2 Change team.

The Foundations 2 Change team all have lived experience of the criminal justice system, with decades of recovery from addiction, offending, homelessness, and mental health. We know first-hand the internal triggers and exterior barriers that kept us stuck within the criminal justice system.

This work supports repeat offenders with multiple and complex needs. Individuals are supported with transitioning into appropriate rehabilitation units, recovery housing, and training/work opportunities.

The rehabilitation model that has impacted many lives over the years, changing addicts into productive members of society. The ripple effect has healed communities and families of all involved.

This work transforms the lives of prisoners, breaking the revolving door cycle of the criminal justice system.

We shared our personal experiences and were able to provide insights into why training like this can be so beneficial for the vulnerable individuals who are in contact with prison and probation services and how much of an impact the correct support can have on their time in prison and their transition when the leave.
