Ware and Rurals PCN
The Ware and Rurals Primary Care Network (PCN) is an organisation dedicated to providing comprehensive healthcare services to the local community for patients registered with Dolphin House Surgery and Buntingford and Puckeridge Medical Practice. As a PCN, we collaborate with various healthcare professionals and services to offer integrated care to our patients.
The PCN has established specialised roles to cater to the diverse health and wellbeing needs of children within the community within the personalised care agenda including a Social Prescriber, Health and Wellbeing Coach, and Mental Health Educator, all of whom play vital roles in supporting the holistic health of children, young people and families.
We are investing more in mental health to reduce pressure on the overall healthcare system in our geography, and working to ensure that children and young people do not escalate through the children and young people mental health services tiering system is a big part of that - and the biggest part of it is ensuring that we continue to train, increase our understanding of suicide and prevent it.
Ultimately, our goal is to ensure that every child, young person, and family in the Ware and Rurals PCN receives compassionate, comprehensive, and personalized care. Joining the ZSA as a member strengthens our commitment to achieving this goal by fostering collaboration, improving access to resources, and enhancing our ability to provide exceptional healthcare services tailored to the specific needs of this important demographic.
