Patients Know Best is a digital tool designed to help health and social care providers bring together patient data. This allows patients to login to access all of their data, from appointments, test results and care plans. 

The Patient Knows Best tool has:

• Full access and control of health records
• Includes secondary care service records and care plans
• Ability to invite family / carer access
• Co-editing and co-produced care plan functionality
• Symptom tracking and mood journal
• Device integration – exercise / sleep tracker
• Patient chooses who to share their record with – including non Patient Knows Best organisations and out of area
